We have a pretty Agile environment and I have tried and am trying to push XP practices, but there has not been a full take-up due to (imho)

• too busy cutting the wood to sharpen the saw
• it is felt that refactoring in C++ is too expensive
• no time to build a suite of unit tests for the current code base

Also if it ain't broke don't fix it and our current ad-hoc process is delivering reasonable results so all else is speculation.
